Our Approach

We aim to listen carefully, reduce complexity and meet you where you are.

At Form and Function, We believe being heard, understood, and respected is foundational to healthcare. Our role is to serve as your expert guide, helping you make informed decisions about your body and your healthcare.

    MDT, Our Clinical Standard

Mechanical Diagnosis and Therapy® (MDT), commonly known as the McKenzie Method®, is a gold-standard clinical system widely trusted for quickly diagnosing and treating spine and extremity pain. Fewer than 2% of chiropractors achieve MDT certification. MDT is not a technique, but a validated diagnostic methodology. It emphasizes repeated movement testing, symptom response, and functional classification to determine the most effective plan of care. All providers on our team are MDT-certified, ensuring a consistently high level of evidence-based evaluation and patient-driven care, regardless of which clinician you see.

    Our clinical process

How a visit at our clinic will look and feel.

    1

    Listen & Understand

We start by hearing your story. Your history, your goals, and the full context of what you're experiencing. This grounds everything that follows in what actually matters to you.

    2

    Thorough Evaluation

Through a complete physical examination, guided testing and clinical reasoning, we observe how your symptoms respond in real time. Our goal is to reveal what's driving your problem, not just where it hurts.

    3

    Structured Plan

You leave the first visit with a clear explanation of the provider's thoughts and understanding of your condition, in addition to an initial strategy. Each step has a reason, and you'll understand the why behind it.

    4

    Progress & Adapt

Your care evolves with your response. We reassess constantly to adjust care to how you're progressing.

What this means for your care

Our universal goals for your care.

    Fewer unnecessary visits

Care is based on your response and progress. We hope you get better faster.

    Clear explanations

You'll understand what's happening, why it's happening, and what we're doing about it.

    Collaborative care

You're a partner in your care. Treatment is adapted to your goals, tolerance, and stage of recovery.

    Tools you can carry forward

Alongside hands-on care when helpful, we share education and strategies that support confidence, resilience, and quality of life between visits.

Who this approach is best for

Our care model works well for patients who want detailed understanding and a collaborative role in their recovery, whether that starts with hands-on care, symptom relief, progressive rehab, or a combination over time.

    • Symptoms that haven't responded to prior treatment
    • People whose symptoms have made them feel afraid or nervous
    • Determining whether surgery is the right option for you
    • Those dealing with chronic pain who feel hopeless

    • Those looking to collaborate in shaping their own recovery
