For Healthcare Professionals

    Diagnostic clarity. Faster direction.

Form and Function is a trusted referral partner for many, from personal trainers to orthopedic surgeons, with experience coordinating complex musculoskeletal cases as part of an interdisciplinary care team.

    When to Consider a Referral

Our clinic functions as a diagnostic and decision-making resource, not a destination for indefinite treatment.

    • Unclear, persistent, or conflicting musculoskeletal presentations needing structured mechanical assessment
    • Patients seeking to delay or avoid surgery through evidence-based conservative management with defined endpoints
    • Prior treatment that has plateaued or produced incomplete answers
    • Need for rapid determination of the most appropriate next intervention

    What to Expect

Efficient evaluation and clear direction, not prolonged or open-ended treatment.

    • Clear diagnostic impression and care pathway recommendation after the initial evaluation
    • Fewer unnecessary visits through targeted, assessment-driven care
    • Patient equipped with self-management strategies where appropriate
    • Appropriate escalation when conservative care is not indicated or not progressing
    • Structured communication back to you at intake, decision points, and discharge or escalation

    Presentations Commonly Evaluated

A sample of the musculoskeletal presentations we routinely assess and manage.

Chronic pain with unclear mechanical contribution
    Radicular or referred symptoms
Patients considering whether surgery is the right option
    Extremity conditions with diagnostic uncertainty
Post-surgical patients not progressing as expected
    Recurrent or non-resolving spine pain
Stalled rehab or incomplete prior workups

    Our Assessment Process

Extended evaluations (60 to 75 minutes) built around Mechanical Diagnosis and Therapy (MDT), designed to classify the presentation and determine the most appropriate next step.

    • Repeated movement and loading tests to classify by syndrome and directional preference
    • Classification directly informs treatment approach and prognosis
    • Screens for mechanical, neurological, inflammatory, and non-musculoskeletal sources
    • Patients are directed toward continued conservative care, injection or medication management, advanced imaging, or surgical consultation
    • When findings fall outside our scope, we communicate that clearly and promptly
